USAG Hawaii is now issuing the Survivor Access Card to eligible Survivors, please see information below:
The Survivor Access Card is now being issued through the Army’s Automated Installation Entry (AIE) system to eligible surviving family members. The card is renewable every three years. Please see attached IMCOM Form 44 (Application for Survivor Access Card). The Survivor Access Card is intended for recognition and installation access (attend outreach events, ceremonies, view memorials). Note: The Survivor Access Card does not provide Army and Air Force Exchange Services (AAFES) or Defense Commissary Agency (DeCA) privileges. For Frequently Asked Questions, Refer to page 3 of the application.
Who is eligible? Eligible surviving family members are those who meet the criteria to receive the Gold Star Label Button or Next of Kin (NOK) Lapel Button including widow or widower, parent, child, stepchild, child through adoption, brother, half-brother, sister and half-sister and step-siblings. Surviving Family members who already have military identification or Common Access Card are not eligible to receive the Survivor Access Card.
Steps to Apply for the Survivor Access Card:
1) Complete Section 1 of the Application for Survivor Access Card, Print Name, Sign and Date.
2) Submit completed application to Survivor Outreach Services (SOS) via In-person (SOS Office), U.S. Mail or Email

The completed application will be submitted for processing and vetting. Once cleared, the Survivor will be contacted on where to proceed to obtain the card. A photo will be taken at the Visitor Control Center on Schofield or Tripler (the card will be generated once your photo is taken). Please ensure to bring two forms of acceptable ID in original form at the time of Survivor Access Card issue.

- If you’re having other portal issues, call 1888-TRIWEST for customer support
- TriWest is aware that some beneficiaries are seeing an incorrect catastrophic cap or deductible information in their TriWest portal and are implementing a fix. Please remember to check back on your own personal situation in a few days.
- Dependent information will be available on the TriWest portal later in January.
- Remember to check on all referral appointments that have been previously schedule. This will help ensure that you have the coverage and are not going to have appointments cancelled.
- Due to the contract change, Electronic Funds Transfers to Tricare could not be switched over. You will need to go to your account and re-enroll in the Electronic Funds Transfer option for payments to be successful.

Purpose: Provide long term case management and support to eligible Survivors and ensure a seamless transition from the Casualty Assistance Officer to the SOS Program.
Mission: Build a uniformed program which embraces and reassures Survivors that they will be continually linked to the Army Family for as long as they desire.
Location: USAG Hawaii SOS is located on Schofield Barracks in the Soldier Family Assistance Center alongside the Casualty Assistance Office.
