🏀 CYS Youth Sports: 2027 Basketball & Cheerleading Season
Sign up to join the basketball team or cheerleading squad! The CYS Youth Sports & Fitness program helps your child develop skills, build confidence, and learn the fundamentals of teamwork in a fun and engaging environment. This program is open to all DOD ID cardholders.

| Eligibility | Children with birth years 2012-2022. Must be 5 years old by the first practice date. |
📋 Registration & Requirements

CYS Registration: All participants must be registered with CYS. First-time and renewing participants can register at Parent Central Services or online with Web Trac.

Medical Forms:

A current Health Assessment/Sports Physical Statement (HASPS) is required for all participants.

The Exceptional Family Member Program (EFMP) form is required.

A Medical Action Plan (MAP) is required if the participant has a medical condition.
